After childhood surgery for testicular torsion, hydrocele and an undescended testicle, doctors warned James*’ parents that there was a small chance he might face difficulties fathering a child naturally in the future.
But it wasn’t something James thought much about until, after ten years together, he and his partner hadn’t been able to conceive and started investigating possible causes.
After months of tests and waiting, they were given a devastating diagnosis: James had non-obstructive azoospermia, meaning no sperm were present in his semen. The chances of conceiving a biological child were extremely low.
What had always felt like an inevitable part of our future suddenly seemed out of reach

Finding a way forward
For a long time, the couple believed their only options were donor sperm or adoption. The journey was emotionally draining – marked by long wait times, uncertainty and difficult decisions. But eventually, James was told he might be eligible for surgical sperm retrieval (SSR).
When offered a choice of clinics through NHS funding, they chose King’s Fertility. Despite living on the south coast, they were drawn to the clinic’s strong reputation and convenient London location. Online reviews helped build their confidence and their first visit took place in early 2024.
By May, James was scheduled for surgery.

at ease - not easy given how daunting the procedure feltJames
From the administrative team who helped with logistics, to the nurses, anaesthetist and consultant who carefully explained each step, James says the support he received made a difficult process more manageable.
Although the chance of finding viable sperm was slightly better than initially thought, the odds remained low. James and his partner braced themselves for disappointment.
Instead, they received life-changing news. After the procedure, the surgeon entered the recovery room smiling – they had retrieved nine vials of healthy sperm.

Looking back on the journey
James recovered well from surgery, supported by the King’s Fertility team. The sperm was frozen and transferred to their local clinic for IVF. By June, the couple had their first appointment. Treatment began in September – and by October, they had a positive pregnancy test.
Now, with their healthy baby boy in their arms, James reflects on his experience:
Although going through all that was the most challenging and painful time of my life, I wouldn’t change any of it. I feel I’ve learned so much from the experience - and that it’s prepared me to be a better, more appreciative parent than I might otherwise have been.
